Along with the bar-coded submission form, you must submit your official mail in certificate and a copy of the original cash register receipt, showing store name and qualifying purchase price circled, dated between March 1, 2010 and December 31, 2010 for Special Golden Estrus® 4 FL OZ with one of the following UPC numbers: (024641140543) or (024641040546) purchased.

All documentation for a rebate submittal must be included in the same envelope. Incomplete or missing information will cause the submission to be disqualified. Please keep copies of all documentation for your records including proof of mailing.

A traceable mailing method is highly recommended.
